Having some issues

I am not sure if it is my oven or what but I have tried to bake this bread twice and both times it has failed. The outside is not akin to "crusty bread" it is rock solid and (I'm sorry it's the first thing I thought of when I saw it!) it looks like house masonry. The inside is doughy and has very few air pockets. I followed the instructions on the box and let the dough rise to double it's size. Pan of water in the oven, preheat to 400º, the whole shebang. I live in central Texas, any ideas?
